Joint Doctor of Audiology Program
Thank you for your interest in the Doctor of Audiology (Au.D.) collaboration between University of Wisconsin-Madison and University of Wisconsin-Stevens Point. The following information provides detailed instructions for applying to our joint AuD Program. Please pay careful attention to the requirements for transcipts and GRE score. These items are required to be submitted to both universities. The following application process will be considered by the joint AuD Admissions Committee. All applicants will be notified of their admission status during the spring semester. It is the applicants’ responsibility to notify the AuD Admissions Committee of any changes in contact information.

- Application Fee
A non-refundable application fee of $56 ($60 for International applicants) must be submitted with the completed Admission to Graduate Study form. The payment should be made by personal check or money order, drawn on a U.S. bank. Please do not send cash. The personal check or money order should be made out to 'UW-Stevens Point'. (Please write your birth date on your check, so we can match it to your application.) An additional $50 express courier fee may be charged to International applicants.

- GRE & TOEFL
- Test scores for standardized tests: The GRE is required for consideration of the application for both domestic and international applicants. Every applicant whose native language is not English, or whose undergraduate instruction was not in English, must provide official scores from the Test of English as a Foreign Language (TOEFL).
- GRE and TOEFL scores should be sent to the University of Wisconsin-Stevens Point from ETS. Find information for the GRE at http://www.gre.org and TOEFL at http://www.toefl.org.
- Use institution code 1919 to request test scores be sent to UW-Stevens Point.
- The code for the Audiology department is 0602.

- Au.D. Application Form
- The Au.D. Application Form is the program application and must be completed in its entirety and signed by the applicant. The application should be printed and submitted to the AuD Admissions Committee.
- Each applicant is to specify a preferred home campus. A home campus refers to which university (University of Wisconsin-Madison or University of Wisconsin-Stevens Point) the applicant prefers to be located.
- Transcript
- Submit an Official Transcript of all post-secondary institutions you attended including any graduate work. Two copies should be submitted to the AuD Admissions Committee. They can come directly from the institution or from the applicant, but must be in a sealed envelope.
- If your native language is not English, or your undergraduate instruction was not in English, send your international academic records, in the original language accompanied by an official English translation. These documents must contain the official seal/stamp and official signatures of the issuing institution. You must also send a set of descriptions of the the courses listed on the transcripts and/or certificates.
- Statement of Professional Intent
- Your statement of professional intent should include why you want to enter the profession, and why you think you will be successful in this profession. State your professional interests and goals. Please sign this letter.
- Letters of Recommendation
- The Doctor of Audiology program requires three (3) recommendations to be submitted, at least two of which should come from faculty members who have knowledge of the applicant's academic performance. The third recommendation can come from a clinical supervisor, employer, or other individuals who have demonstrable knowledge of the applicant's academic potential and likelihood for success in graduate school. Please use the Recommendation Form and any other documentation should address academic performance, motivation, personality and potential. All recommendation materials should be submitted in a sealed envelope with the recommender’s signature across the flap.
